I have seen several ENT physicians and am now with a speech therapist.  I have tried medications for acid reflux.  This has continued over a period of 5 to 6 years with no relief.  It seems the latest is tension. It does get so bad at times that I simply cannot talk or don't want to talk.

Hi, sorry to read of your discomfort.  Have you tried ENT or allergist ?  I've read a few complain of hoarseness, due to allergies.  Acid reflux can cause hoarseness due to acid corriding the throat.  Best thing to do is continue to seek a physician, till you discover one that can resolve your complaint.

As for thyroid, I had a very large one, did not have any voice difficulty, only swallowing/breathing.  I remember constantly swallowing, to readjust my throat.  Have you seen an endocronologist?  If so, what did they state.
