Thyroid disorder/nodules

noted labs {TSH 0.257, Ferritin 31, WBC & Platlets elevated at times, RBC ok, H & H ok} had abnormal boderline thyroid lab for 2+ years.  Grandmother pasted away from ovarian Ca , her sister had brain tumor, father may have had CA ?
mom & aunt had ovaries removed early on
I have 4 childern { set of twins} 36 yrs old had gastroplasty in 2002 lost most of the wt. , always had fatigue, & weight problems at age 19 had mono. , Bell palsy 30 yrs old, DX:Degeneration of the neck and spine in 2000

s/s old {migranes, lymph swelling, before leg pains at night, abnormal swelling to hands and feet at night, tachcardia, feel cold,Breast cysts and swelling multiple, fatty deposits to lower back and spine, IBS.severe constipation with bloating/gas sharp pain even to rectum } had colonoscopy and endoscopy no abnormal findings only IBS

new s/s {recent neck pain, lymph swelling,sore-throat, hacking cough, chest tightness, upper back pain, right side breast/ chest wall sharp pains at times, low-grade fever, wheezing,bronc-spasms, recent with sickness lasted 2+ weeks 2 rounds of ABT}  

sono: ultrasound of the thyroid gland reveals a 1.2 x 1.2 x 2.4 cm hypoechoic nodule on the left. Doppler examination reveals it to be hyperemic. It is amenable to ultrasound guided percutaneous biopsy if clinically indicated. Inferior to the noduleis a 5 mm hypoechoic nodule or cyst. On the right the is a 2 mm hypoechoic nodule or cyst.  The right lobe measures 4.3 x 1.5 x 1.7 cm. Thyroid isthmus is thickened to 7mm.

What does this all mean?

Hypoechoic nodule mean & hyperemic?  & isthmus is thickened to 7mm what is normal?

Should I be concerned about having other Bone scan test?

Thyroid biopsy are you awake,  is it a day surgery and is it painful? Do you have an problems usually after this kind of biopsy? How long does it take usually to get your results back?  Can the surgeon usually tell what kind you might have even before the results come back?

Hypoechoic nodule means it reflects less of the ultrasound waves then the rest of thyroid; hyperemic means Ibelieve increased blood flow ; due to its size and features biopsy is necessary; it must be done under theultrasound to assure that the nodule tissue is picked in the sample not just blood; the normal isthmus is 3 mmm yours is about 2 times thicker; the right lobe is normal size.
