thyroid levels

I had a large mass on the right side of my thyroid.  Doctor removed right side and one parathyroid gland (no cancer).  I have high insulin (taking 500 mg metformin 2x daily).  Doctor put me on phentermine for weight loss (no help).  I just had my thyroid tested since surgery. My Free T4 is .96 and TSH  12.74.  Insulin still at 16 while on Metformin.  Sodium 138, Potassium 4.0, Creatinine 1.0, Calcium 9.4, Random glucose 89.  Doctor put me on Levothyroxin 75 mcg 1x daily.  What is the normal level range for the Free T4, Free T3, and TSH and Calcium, Creatinine, Potassium, Sodium, and insulin?  I feel like **** since they removed my thyroid.  Tired all the time and cranky.  Can't lose weight no matter what I try and I have tried every diet under the sun along with regular exercise up to 4 hours every day.  I just got the prescription for the Levothyroxin today, so I will start tomorrow.  Do you think there could be another cause in addition to the thyroid that is preventing me from losing weight?  Hypothalamus, adrenal, pituitary glands?  Should I have any of these checked?
I also have polycystic ovarian syndrome (reason for metformin).  I'm tired of Doctors that won't listen to me.  I have been to several Doctors over the years and they tell me there is nothing wrong with me.  One even told me that my problem is that I don't leave the kitchen.  I eat 300-500 calories per day and zero carbs per day.  I work out AT LEAST 2 hours per day, one hour weight training and 1 hour cardio.  Something is wrong with me!!  Please help!!

Weight gain directly from thyroid tends to be water and isn't extreme. You should be investigating other health issues.
Being at a good level of TSH will make it eaiser to lose weight, but not necessarily that much easier. Dr. Lupo indicated that a diet of 1200 calories a day and 4.5 hours a week of exercise is most effective.

Weight gain is part of thyroid disease
Being healthy is better than going on a very low calorie diet, which will make you lack important nutrients.
Thyroid disease affects Metabolism which is why burning fat is harder etc.

More gentle exercise ...a longwalk each day etc is far better than cutting back on essential food.

Thyuroid meds will not make you lose weight.
But, when the meds make you well again your weight should return to your pre thyroid weight...this can take time dont expect miracles.

It is horrible I know...but we have to live with it.

Many women with PCOS seem to have weight issues. Obesity, or inability to lose weight, is listed as a symptom of PCOS.
I 'm not sure the thyroid med will be all that helpful for weight-loss, not from what everybody on them is usually saying.

I think you should be eating more than  500 calories a day.
