Since you've been to so many doctors, and had so many tests, I have to wonder if it's caused by something you're being exposed to, either willingly or unknowingly. Maybe you're going into a building with mold? Do you work in different locations from time to time, maybe? Visit a friend's house that might have mold?
Maybe you're eating something that you have an allergy to. When you feel it, track everything you've eaten (or better yet, start tracking all your food, so that when symptoms start, you have a record to look back through.) There are a bunch of apps that you can use on your phone to do this.
Do you alternate shampoos? Maybe use a clarifying shampoo or conditioner every now and then? Shower at the gym? Think of everything - all your patterns and variations from them.
This started when you moved to Arkansas. Are you in a house that's been flooded before? (Arkansas has had flooding, if I remember correctly.) Could your home have mold in it? You can buy a mold testing kit at the hardware store, or call someone in to test. I would google your area and flooding and see if you can find out if your area of Arkansas has had flooding. If it hasn't, that doesn't mean you don't have mold - a leaky pipe can cause mold - but it's a place to start.
