chronic abdominal pain

i have had chronic abdominal pain since the beginning of april and i have been diagnosed with endometrosis endo f last year and the gynae have changed my treatment from provera to zoladex to see if that would help but hasnt yet and should have worked by now, not sure if it is caused by endometrosis casue had a CT and showed that i have a number of lymph nodes in my bowel and one that is very prominent in my RIF measuring 16mm x 8mm, i think that it is the lymph nodes causing the pain but GP thinks not...

dont have symptoms of GERD

having 3rd zoladex implant on thursday

casue im going away for a week its a couple of days early

have to go back to see the consultant when i get back and have to have the full 6 implants before they will think of doing another laparoscopy.

have tried panadine forte and doesnt help just puts me to sleep and cant use tramal casue im on both endep and lovan for depression so it doesnt leave much i can use for pain relief...

Hi,

I would suggest that yo try some pain relief medications for your pain and see if it helps. Do you have symptoms of GERD?

The multiple lymph nodes would need a follow up - but they are not enlarged in size.

You need to get a hormonal level done and check on your doctor about the medications you are on and the endometriosis.

The weight gain and nausea is most probably a side effect of the medications you are taking - let us know about how you are doing and about what your doctor advises.

below is the abdo CT report
exam date: 5/6/08
History: Right Iliac Fossa pain 8 weeks. Tender RIF. Past history cholecystectomy and appendicectomy
Technique: Oral contrast was administered, no IV contrast due to difficult venous access.
Findings: Images through the lung bases are clear.  Cholecystectomy clips are noted.

Prominent lymph nodes are demonstrated throughout the bowel mesentery, the most prominent in the right iliac fossa measuring 16mm x 8mm.  There is however no evidence of stranding or inflammatory change.

Further subcentimetre adenopathy in the para-aortic regions are noted.

There is no evidence of hydronephrosis or renal calculi.  Incidental splenunculus.  No abdominal collections or masses.

Comment: No focal abnormality is demonstrated. There are numerous subcentimetre prominent lymph nodes throughout the mesentery, the largest of which 16mm x 8mm in the right iliac fossa, usually these are of dubious significance but due to the number warrents futher follow-up, suggest a repeat CT in 3-6 months to exclude enlargement of there nodes e.g. in the setting of lymphoma.


am due to have follow up CT in september.

end of last month had a colonoscopy which was normal and no findings.

TREATMENT HISTORY
implanon inserted in october 2006
started provera 10mg in august
laproscopy diagnosed endometrosis end november
increased provera to 30mg beginning of february
changed from provera to zoladex beginning may

have seen gastro enterologist in past regarding pain and they had no idea wat was causing the pain...

hope this helps...  still have pain, weight gain and nausea.

Hi,

Could you post the abdominal CT report here? Are you having any other symptoms - weight loss, loss of appetite, nausea , vomiting, etc?

The endometriosis could be one of the reasons of the abdominal pain - but the lymph nodes you mention are not a normal finding and this needs further investigation to find out the cause of the enlarged lymph nodes. These could be compressing some structore to cause pain.
