On and off Severe penis pain

Hi I am a 19 year old male yes I am over weight but I do not have any medical conditions besides asthma(born with it). I have this pain in my penis only around the tip and on i guess you’d say the back side underneath the forskin but no where else besides them two spots and it starts out like annoying for about 30-45min then progresses to me in full blow tears every time the pain is the same every time. Also sorry forgot to mention that while that’s happing my testicals have this dull acking pain. The pain comes and goes randomly and I don’t take anything to ease the pain because nothing helps and being nothing helps I learned that the pain lasts between 3-5 hours each time. I have dealt with this pain for I’d have to probably a good 17 plus years of my 19 years being alive basically as far back as I can remember lol. I’m been getting this pain at least every two week sometimes just days a part. When I was younger I went to the doctor for it he said drink cranberry juice(love the stuff now) and shower don’t take a bath so I did and I still had the pain no matter what and so because of it I regularly went to the er at any time of day because of how much pain I’m in. The hospitals yes a quite a few of them only asked me about how the pain was and where it was and all that but never did any tests and just assumed it was a uti and went here you go have this course of antibiotics and you’ll be okay well need less to say it never did “go away” it always came back. I did have a urine and blood test done was came back negative for everything they tested for I even saw a urologist and he just sent me away saying it was nothing. I have no clue what to do all I know is that I’ve lost some of my sanity cause of it and I can’t handle it anymore. Please help! Also please do redirect me to other pages I’ve been search this site for 3 hrs straight now and can’t find anything I’ve googled it at least a million times and can never find anything other than UTI UTI UTI UTI UTI it driving me insane!
