Help for SRPE - baclofen not working any more

For the past decade I've had "SRPE" - ("Patients with SRPE will be awakened by painful erections during sleep, which affects their sleep process and general health. " -  https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8749065 ) which cause me to never get more than 2 or so hours of uninterrupted sleep a night. Two years ago I was prescribed Baclofen for it which worked great at first but keeps requiring more and more to be effective - I'm now at 60mg a night and seeing unpleasant muscular side effects from it (and it's just not even working much any more).

My urologist has admitted he doesn't know what else to try. Any ideas?
