Long Term Urinary Symptoms, ED, and Penile Bruising...it is Prostate Infection...or could it be something worse?

I am a healthy, STD free (recently tested) 20 year old male. I was initially diagnosed with an acute prostate infection in October. I was given Bactrim (Trimethoprim Sulfatrim DS 160-800MG) and believed I was cured. Symptoms have come back since (Around November and most recently I was given the same prescription. Most urinary symptoms have gone away but there is still an urge to urinate almost constantly with nothing coming out, constant perennial and testicle discomfort and pain, and feeling that something is blocking my urethra because my urine sometimes splits into different streams. I also have been having problems with ED for a few months which has been a major problem in my relationship. I've been to the doctors multiple times and they have been little help. I can masturbate to ejaculation but have had problems reaching an orgasm. I feel as though my pelvic floor muscles are always tense as I try to do kegel exercises but can't relax the muscles afterwards. My penis seems to tense (without my having an erection) on it's own and it almost seems to spasm when I relax it. The Bactrim has alleviated most of the symptoms of what must have been a prostate infection but I am still in pain.

A more recent (or recently noticed) issue has been that the vein on the underside of my penis is somewhat reddened and irritated and there is what looks like a bruise there. The skin on the shaft of my penis is a bit more slack than normal (I'm circumcised).

I am seeing a Urologist soon but any help you can provide would be really helpful as I'm concerned there are no obvious physical symptoms I will be sent home with nothing.

Any help would be helpful.

I've done research and believe it may be a problem with the Bulbospongiosus muscle. I think it may be the problem specifically because I have problems with 'pushing out' the last drops of urine and because of the erectile dysfunction and inability to orgasm. I've also sometimes had problems even while masturbating where I can expel precum and some ejaculate but don't feel as though I've emptied everything. Is there anything I can do if I've somehow had a nerve/muscular injury?

I should also mention that there is a bluish purple discoloration on top of the edge (closest me) on the head of the penis that I worry is form lack of circulation. There is pain/tenderness along the length of the perineum and when pressure is applied underneath the testicles there seems to be an enlarged lump.
