Dull ache in scrotum and perineum

Hi,

I am a 22 year old male and for the past three weeks I have been experiencing some "aches" in my scrotum and around my perineum. (I say perineum because that is generally where I think it may be located, it is deeper though... for some reason I keep thinking its the prostate.)

The ache seems to move around and is not constant. Last week it usually felt like it was in the back of my testicle (but not actually on the testicle) and this week I don't have an ache there. However, I do feel it again in my perineum area. This tends to worsen when I urinate (not actually during urination but after).

I believe I am urinating much more frequently although it could be because I am paying more attention to my urination patterns. I've been able to regulate the occurrence of the ache somewhat by (1) urinating and not trying to force out urine at the end (because it always feels like I have to continue to urinate) and by not masturbating, which I can't really confirm caused the aches or not.

Anyways, this is really worrying me. Sometimes when I sit down for awhile (or wear a tight belt) I get the pain in my perineum again. When this happens I get a dull ache down my leg as well... not a pain but rather a discomfort.

Please let me know what you think. I am in a new city and working on finding a doctor but in the meantime I'd like your opinion. It's a rather embarrassing and worrying topic and would like to be prepared for when I go into the doctor's office.

This definitely sounds like inflammation of the Prostate, or 'Prostatitis'. I've had it & many of the symptoms you describe sound pretty similar. Don't be embarrassed - this is actually pretty common & does tend to occur in younger, sexually active guys. It can start as a bacterial infection that settles in that area, but there's also non-bacterial prostatitis that is just an inflammation without detectable infection. Some Urologists believe that inconsistent sexual activity contributes to developing this type - in other words, if you were to go long periods without ejaculating, then a period of intense activity (like with a new sexual partner or could also be from a period of very frequent masturbation), that could trigger it. The thinking is that the prostate benefits from regular but not excessive sexual activity - a few times per week. Anyway, start with your regular MD, if that doesn't help you, see a Urologist.

I have the same problem. Initially it was extreme pain and discomfort in the perineum, that worsened when I urinated, and stayed 'flared up' for up to an hour afterwards. Urination and that area feel the worst when urinating after masturbation. Its almost like somethings 'clamping' my urethra too. Ive done 6 Months of physical therapy, and its gotten to be manageable, I can do things now. BUT, the constant radiating(I would now consider it discomfort, rather than pain) discomfort in my perineum still remain, 24/7. Peeing is easy and normal(and almost relaxing, like it used to be) sometimes, but others its just like it used to be. Going to another Urologist to see whats going on, why the hell cant they figure out whats wrong with me? Your not alone.

I have exactly this, pain radiating back and forth. Did youse ever find out what caused this? If so any treatment which worked?

Exactly what I have.  Haven't correlated it with urinating, but I have this perineal thing going on too.  It's sometimes my testicles, sometimes my perineum.  Did you ever find out what was going on?

How in the world could you possibly state this person has Chlamydia.  You know there is enough anxiety going around without someone speculating he has Chlamydia!!

There are plenty of other things it could be!  Trust me I have been through the ringer for similar symptoms myself!

You have chlamydia. Get treated quickly. Tell your ex girlfriends. If they have it and don't know, it could ruin their ovaries (no kids). It can ruin your balls too.

Hi,

Hmm, that is hard to say. There isn't a pain when I urinate, per se; however, I have noticed on several occasions that the urine felt very warm (warm enough to notice) as I urinated. This isn't frequent though and I have felt this in the past, which is normal I suppose. The ache's have lessened quite a bit since my last post. They only seem to arise if I urinate (explanation below) or get an erection that lasts for more than 5-10 minutes.

What brings on bladder infections, cystitis, prostatits and epididymitis? What types of medication are used to treat these? I am still trying to find a doctor. I've asked for recommendations from my previous doctor so hopefully I will be able to see one shortly.

I had started to examine how it was that I was urinating, since this seemed to be the main source of heightened aches. I noticed that I was actually forcing urination at the end since it felt like I still had to urinate. I've made efforts to stop this and ever since I've noticed a decrease each day in the frequency of the aches... not sure what that means.

Hi.

I do understand your concern. I suggest that you have this assessed by a urologist. At this point differentials will be a bladder infection or cystitis, a prostatitis or epididymitis. I know you are highly anxious about this but pain in the perineal area and also involving the testicles may involve the prostate or the epididymitis.

A baseline urinalysis with urine culture may be necessary. Your physician may also request for a prostatic secretion culture which may help determine an infection involving the prostate.

Do you have any associated pain with urination or with sexual response?
