Urethroplasty information

I would like to share my personal experience with urethroplasty, as I have noticed many questions about this topic here.

Almost 2 years ago, at age 37, I had urethroplasty for what was a complex obliterative stricture of 3-4 cm length in the bulbar urethra. The technique used was augmented end-to-end anastomosis using buccal mucosa graft. Surgery was done in Arezzo, Italy, by one of the best reconstructive urologists in the world, Prof. Guido Barbagli.

I stayed in the hospital for 5 days after the surgery and wad on bed rest for 1 month overall. A month after surgery the urethra catheter was removed and the flow measured. Results were excellent, 35ml/s max flow rate. There was substantial post void dribbling, requiring me to press slightly under the scrotum in order to expel residual urine from the urethra. I do not consider this a problem though, given the very good results.

I did not have sex for 3 months after the surgery. Erection was definitely weaker at that time but has been improving since and I dare say that a year after surgery it had recovered almost completely (or at least 80% of what it was). No pain or other symptoms during intercourse.

Urethroplasty is a technically demanding surgery and is considered a reconstructive surgery, so the term pretty much says it all. Despite any hesitation or fear, due to the sensitive area it involves, it has the highest chances for long-term cure of urethra strictures, particularly in the bulbar urethra. However, if you choose to do it, be absolutely certain that you get operated by a urologist specializing in it, as the urethra is an "unforgiving" organ.

surgery is not painful, keep in mind that the post-operative at home even if you can walk out and do anything you want (if you do not mind to go for a regular evening stroll with the catheter bag on), but basically the time flows  being cared and cuddled in bed.

The first 5 days postoperative you are required to stay in bed and the wound if you do not touch, you won't even feel it.

The thing that may give more trouble, but it is subjective, is the wound in the mouth, which requires two to three weeks (the time needed for all the points to fall) before returning to some normality, all the rest as described by gazis, Sex after 3 months, after some months or 100% erection (this is subjective).

In essence it is not a walk, but better to act before they carried away the problem.

The center of Arezzo is great, as an alternative are very skilled in these techniques in Padua.

l'intervento non è doloroso, tieni conto che il post-operatorio anche se puoi camminare uscire e fare tutto quello che vuoi (se non ti dà fastidio andare a fare lo struscio con la sacca del catetere addosso), ma sostanzialmente il tempo lo passi a letto curato e coccolato.

I primi 5 giorni post operatori li fai stettamente fermo a letto e la ferita se non la tocchi, non la senti nemmeno e non da dolore.

La cosa che puo dare più fastidio, ma è soggettivo, è la ferita in bocca che richiede due-tre settimane (il tempo che cadano tutti i punti) prima di tornare ad una certa normalità, per il resto tutto come descritto da gazis, sesso dopo 3 mesi, erezione 100% dopo qualche mesetto (anche questo è soggettivo).

In sostanza non è una passeggiata, è un intervento pesante, ma meglio agire prima che trascinare troppo il problema.

Il centro di Arezzo è ottimo, in alternativa sono molto abili in queste tecniche anche a Padova.

This is a really old post from April of 2010, so I'm guessing this guy is log gone by now.

Your question about the surgery and what the most painful part is: In my experience, it's not the surgery itself that hurts and causes pain, it's the recovery AFTER the surgery.

"Open" surgery requires cutting through muscle, which is quite painful when healing.

Has your urologist indicated whether he'll be performing the procedure "open", or Laparoscopically?

I've just been told that I need to have this surgery done and the idea is of the operation is absolutely scary.  Is it painful and if so what is the most painful part???
