ejaculation change in one day

hi

i normally ejaculate a good amount, over a year ago, my age then was 28 in the UK, i was masterbating and suddenly when i was about to ejaculate, i felt pains and 2 or 3 tiny drops of ejaculation came out along side it alot of testicular pain, it felt like a blockage, few days later i tried again and nothing came out when i orgasmed? as weeks months went by, i noticed the semen as watery, somtimes with yellow tints, this got better, i get the same symptoms intermittanly, my ejaculation volume now remains 20% of how much it used to be, i get ALOT OF testicular, pelvic pains after ejaculating.

i did go to a urologist, sent me for a  transrectal ultrasound. The nurse couldnt continue the procedure as i had hemorrhoids and it was too painful and made me bleed, so the nurse stopped the procedure (ultrasound device was very large). At the consultation urologist said everythings fine and discharged me (nothing was checked to do with my symptoms), i didnt go back because i had the bad experience and fear it. Only thing that was done was a semen anylisis which came back fine (without whitecell or bacterial check up).

am i suffering from some form of obstruction? as everytime i try to ejaculate its very little or a few drops and is ever so painful after?

what should i do? can i complain about the serviCe (nhs) ? what should i need to check for obsruction?

thanks for the reply, very helpful, i have a few more questions regarding the procedures:

Can MRI provide better imaging of the ejaculation system?
Can a CT  scan be used instead of a MRI?
Does the transrectal aspiration procedure need to be done along side the MRI or is that a seperate procedure involving ultrasound and a needle? how is the trans rectal-aspiration done?

Can you tell me the treatment for obstruction of the seminal vessel, vas deferens any of the ejaculation system (other then infection treatment with antibiotics)? i cant find much online and it seems there's very little options with high severe complication risks with treatments?

i will have to go through my dr to refer me back to the urologist, this will be the 2nd time, my 2nd time referal was rejected by the urologist a few times, he doesntr feel i have anything wrong and has done no tests, would it be better for me to go through a complaints procedure?

Hello,
From the symptoms the possibility of any obstruction in the ejaculation system cannot be ruled out. Transrectal ultrasound has a relatively low sensitivity and cannot diagnose ejaculation obstruction accurately.  So you can talk to your urologist and get MRI scan and transrectal needle-aspiration of the seminal-vesicles done.
