MEDICAL PROFESSIONAL
Hello,
Presence of visible blood clots in urine is called gross hematuria or macroscopic hematuria. Most often it is related to urinary tract problems like kidney stones, inflammation of urinary tract, prostate enlargement, renal vein thrombosis, certain kidney tumors and polycystic kidney disease, blood coagulation disorders and anticoagulants.

In your case, most of the tests have come out normal, hence consultation with urologist is recommended to confirm the diagnosis with help of bleeding time, clotting time tests, urine and semen analysis along with ultrasonography of prostate by trans rectal method.
I hope it helps. Take care and regards.
