Pee Anxiety While Away From Home

My friend, in his early 30s, has no insurance and no job and part of what's holding him back is that he has an issue where sometimes unexpectedly when he drives into town, he has a sudden urge to pee and it can turn into a panic attack especially if he doesn't know where he could go pee. We're not sure what's causing it. His blood pressure is normal. What doctor should he see first? What kind of tests should he get?

He is trying to drive more and more a little at a time to expose himself more to being out and about without issue but had a set back recently. He figures if he goes to a psychologist they'll just tell him the same thing -- go drive more.

This doesn't address the anxiety part, but there are so many remote jobs now. He should address this with his doctor, but in the meantime, he could still have a job and work from home.

If the issue only occurs when he is driving, or otherwise away from easy access to a bathroom, the cause is unlikely to be physical. A counselor is the appropriate route, and the guess about what a counselor would tell him to do is just that - a guess.

If the anxiety stems from fear of not being able to access a bathroom, he might consider wearing an adult diaper while driving. Perhaps having a back-up plan in CASE he really had to go would help him relax.
