Ketoconozole is known to cause severe liver failure, which is probably why your vet is being cautious. Have you experimented with pure coconut oil? It has some excellent dermatological properties when rubbed into the affected area. It's strange stuff, as it's a solid until warmed, but once warmed up, it becomes an oily liquid that is easy to apply. I used this on my rescued greyhound, who had many bare patches of irritable skin when I first got her. Within a week all the patches had started to grow hair again and were massively improved. After a month I stopped the treatment and the patches of irritable skin have never come back. Worth a try.

Yea, and this brand is the one my Vet sells (pushes)... we had our Westie on the Hill's Prescription Z/D which has hydrolyzed chicken protein. Not part of the recall, but Hills none-the-less.
I have my Westie back on 4 Health Salmon and White Fish Gibbles and Salmon canned. So far, about two months, no more problems with Yeast or at least no eating the fur off his legs.
I'd like to have a supply of Ketoconozole tables, but my vet will not give me a prescription beyond what is needed for the period she specifies.
