Any info on Telazol causing the heart to stop

I took my seven year old little girl Peke and her brother to the vet July31st to get there nails and feet done.They always have used Telazol on them to relax them and to keep them comfortable.After she was giving the injection IM her heart stopped within 3-4minutes.The dose was correct,I checked the bottle plus the syringe and the log book.So i know that wasnt the problem.We tried to resucciate her for 1hour and 45minutes and we never got a heart beat back.She was my little girl and i just need some help on why and what could have happened to her.I got her when she was 4 weeks old,She was planned on even before her Biological mommy was pregnant.She was my everything.My husband and boys are really taking it hard to .We just need to know what could have happened.I keep blaming myself  for taking her,But i would've never thought that this could've  happened especially after all the times that she has had it before.Please if anyone knows anything let me know....Chan
