Can someone help me define these symptoms?

Hello All,

I am a 24 year old male, white, spanish born, 1,83 meters and presently 68 kgs.

I always have been healthy asides my asthma and smoking, even competed in mixed martial arts and I went to the gym 10 hours a week besides obliging my professional responsibilities.

Ever after going on a Business trip overseas, I have been feeling very tired and without any motivation at all.

I stopped doing exercise all together as I felt my body shutting down. Slowly I started again my morning workout, only to find out I had to reduce it greatly and even after 1 month and half of a daily routine It's hard and I get extremely tired. Nothing much 500 or so abs and 150 push ups in 35-45 minutes, I used to do this in under 20 minutes.
My appetite has gone down and food doesn't seem good, and I eat a lot. I cannot digest lactose based food anymore, I feel bloated and my digestion goes on for hours, plus the flatulence. Whenever I eat what was half I would eat six months, and I'm full. Headaches every other day, mostly in the right side and back of the head, until now I cannot sleep with paracetamol 1gr.
It's a strange tingling feeling in the back of my head with this odd "piiii" sound on my right ear that sometimes get lower but never goes away.

At the time I assumed it was stress in work, but then I found a lump beneath my ear, just behind my right jaw. I can "grab" it 2 fingers and move it around.

I went to the doctor on the 25th of January and she told me it was an ear infection and I was prescribed Amoxicilline Actavis Disper 500mg 3xday for 10 days, and Xylometazoline HCI Actavis for 7 Days.

The lump, the headaches, the tingling feeling, not being able to fall asleep and when I do I am not resting, a sad and unsociable feeling which can lead me sometimes to tears, the tiredness... Nothing as gone away or seen changes.

I called the doctor again to make an appointment and explained, the assistant said to wait a week or 2 because it may take some time to go down, the lump.

Can anyone help on this? Should I go to another doctor? Should I demand a CT Scan knowing that I had 2  at the head due to sinusitis?

I sure hope you are not smoking while this is going on.

Did the physician order a serum viral panel at all? (these are labs)

How big is this "lump?"  Is it painful?

I am sorry I meant to say "add" instead of "had"... this is so frustrating as I never did these mistakes writing and now they seam common...

I forgot to had the nausea I feel every morning, or after eating anything at all...
