Had my eyes checked twice in 2 weeks and Axes varies +/- 101

I wear prescription glasses though had not had my vision checked since 2012. I went to the eye doctor on 1/4 to have an evaluation and they mentioned my left eye had changed a lot. I went to pick up the glasses today (1/18) and could not even see through the left eye lens it was completely blurry to the point I could not read the dashboard. I went back in the same day to have the doctor double check and they said the glasses matched the prescription they provided on 1/4 however they were happy to replace the left lens as it 'might' have been miss-manufactured.

The doctor did a full exam again and rewrote the prescription; After comparing the 1/18 prescription to the one that was written on 1/4 I noticed the Axis and Cylinder on OS were different from the 1/4 see below:

1/4/2016 Prescription
OD: Sphere=pl; Cylinder=-1.00; Axis=161
OS: Sphere=+0.75; Cylinder=-2.25; Axis=111

1/18/2016 Prescription
OD: Sphere=pl; Cylinder=-1.00; Axis=161
OS: Sphere=+0.75; Cylinder=-2.00; Axis=010


Question:
1. Is the prescription for OD going to be drastically different between 1/18 and 1/4?
2. How does axis and cylinder affect the prescription, could the changes between the two prescriptions explain why the new lens was so blurry to me?

I understand I'm talking about the glasses RX you were wearing prior to 1/4/16  If this is your first pair of glasses then its obvious the RX 1/4/14 was written down wrong.

This likely represents what is called a transitional error.  That is the glasses test 1/4 OS should have been written as 011 not 111   There is no difference a person could tell between axis 011 and 010      If you would have or could get a copy of your "old" prescription I'm pretty sure the OS would be about 010 not 110    Normally you would not be charged for the repeat exam by the eye doctor and not charged for the remake of your OS lens

I referenced the old copy of the prescription that was given to me on 1/4 as well as the prescription that was given to me on 1/18. The OS axis on 1/4 was 111 the axis on 1/18 is 010.
Correct they are not charging me to remake the lens. I was more or less asking if that could explain why the left lens was blurry because the first prescription stated 111 the new prescription states 010, also the cylinder changed from -2.25 to -2.00. .

Correction for Questions: I meant to refer to OS rather than OD as OD did not change at all.
