I would not be afraid. There is no guarantee if you have it, but the prognosis if you don't is likely grim. I had a scleral buckle at the same time as a vitrectomy almost a year ago. The recovery was slow and painful, and I've had lots of other procedures since to deal with complications. But I'm finally getting close to 20/20 vision in that eye and I'm very grateful for what I have.
You have a very serious problem, likely a complicated retinal detachment that has failed to respond to initial procedures. You can certainly get a second opinion from another retina surgeon but its likely that if nothing is down the eye will permanently lose most or all of the vision. These are big operations for ultra-difficult problems that use to go blind before scleral buckles and vitrectomies were developed.
