Its very   sore and fairy large. Itchy at times and painful.  I am incontinent and have been for several years.

They probably could if they are chafing, or you are having a reaction to them. Did you recently change brands or has the brand you use recently changed them somehow, like "new and improved"?

Are you sexually active? Could it be herpes? Herpes doesn't often bleed, but if you are scratching in your sleep, you might cause bleeding.
