Little toe and intense pain walking

One of my son's little toes is causing a lot of pain. There appears to be a blister of sore of some sort. We originally thought it was a lister corn, but am no longer sure. The podiatrist he saw thought the same and shaved it off, but the pain hasn't gone away. It's been several months.
A little background: He had compartment syndrome in that leg and had some serious surgeries. The blood flow in that leg is poor so healing may be an issue. He says it's not nerve damage because he is very familiar with that pain. This is a throbbing pain.

Take a coin, tell the pain to go to the coin and then throw it away somewhere.
