my husband had something similar to this and the doctor suggested the cheapest thing and suprisingly it worked awesome! Rub some Vapor rub, ya know the chest stuff, on really thick at night and put on socks. Sleep that way for a week or so and it helps tremendously. Suprisingly the menthol in it kills bacteria and fungus, kinda smothers it, and those are the two most common causes of foot odor. Good luck.

I used to have some foul foot smells and it turned out I was having a reaction to the glue used in shoes!  I had to use a topical steroid to get the reaction to stop, and then change my foot / cleaning habits.  I've  always been good about cleaning my feet, but I didn't know that polyester in socks can make a difference!

Suggestions - make sure he is using 100% cotton socks.  If you go to sports stores you can gest really good wicking socks that will draw sweat away from the foot.

Make sure he's drying between his toes REALLY well.

A little antiperspirant on the sole of the foot wouldn't hurt (I do that in the summer).

Replace shoes often.  Remove them from the foot often if he can.  Wear season appropriate shoes too.... sandals, flip flops, canvas sneakers in summer.  (As in NO COMBAT BOOTS in the summer!!!).  I also found during the winter I literally had to bring difference shoes and socks to wear at work!  I'd wear my thick warm socks and boots out to the car and to work, and then change my socks and shoes there.  Then change back to thick socks and bots on the way home.

I find real leather shoes make my feel stink WAY less than pleather/fake leather) too.  Make sure he isn't wearing sneakers/enclosed shoes WITHOUT socks - my roommates used to do that and it made us all want to vomit his feet smelled so bad. Once he stopped doing that and replaced his shoes his feet stopped smoking us out of the apartment.

As for what kind of doctor - I would think a podiatrist (foot doctor)  or a dermatologist (skin doctor).  If your grandson is particularly sweaty, his PCP might be able to give him a prescription antiperspirant.

Good luck!
