Protected sex with a csw in India. Had shaven groin area 3 hours before

Hi Doctor,


3 weeks back I had protected vaginal sex with a sex worker. Dont know the HIV status of the partner. Condom was never broken to my knowledge. It was properly used, never gone off during the act although my penis was flacid at the time of starting to use the condom. From that day onwards I was tensed whether I got infected with HIV. A few days back, there was a pain on my right calf muscle. Then on my right arms and now on the shoulder blades as well. Then a slight pain has started on my leg knees over the knee cap and my left ankle joints. I previous had an issue of high uric acid crystals in my blood and joint pains especially on joints

(2 days back my work was very stress due to long working hours. I used to sleep at 1 am or 3 am and wake up at 6:50 am or 8 am. Also I had travelled on a night train with another guy on the same berth. So it was a strenuous travel also.)

But since Yesterday I felt very fatigue. My body aches several places especially when I put pressure on muscles on my forearm and shoulders. Last night I had a mild fever. I had paracetamol. Now I am just okay, but feeling tired or a little dull.

Please let me know whether I am infected with HIV.

It doesn't matter.  You did not need a NAT.  You did not have a risk.  Your symptoms are probably anxiety driven.  What ever the root cause of it is,  HIV  is not your problem.

This answers all your questions and if you think of any others, then you can get the answer if you just reread about the 3.
You had no risk for HIV so a test would be a waste of time.
HIV is instantly inactivated in air and also in saliva which means it is effectively dead so it can't infect from touching, external rubbing or oral activities. It doesn't matter if you and they were actively bleeding or had cuts at the time either because the HIV is effectively dead.  
Only 3 adult risks are the following:
1. unprotected penetrating vaginal with a penis
2. unprotected penetrating anal sex with a penis
3. sharing needles that you inject with. Knowing these 3 are all you need to know to protect yourself against HIV. The situation you describe is a long way from any of these 3.
Even with blood, lactation, cuts, rashes, burns, etc the air or the saliva does not allow inactivated virus to infect from touching, external rubbing or oral activities. Doctors have calculated the risk from what you describe to be less than that of being hit by a meteor, therefore no one will get HIV from what you did in the next 40 years of your life either. The above HIV science is 40 years old and very well established, so no detail that you can add to your encounter will change it from zero risk.
Your symptoms are in your imagination from fear but you aren't dying so you can relax and they will go away.

Thanks for the post. I have a few more doubts
1) why I am having muscle pains - each time at different places - either in right calf muscle or left calf muscle and anyothet place near to joints like muscles near to knees or my forearms.
2) the condom was provided by the sex worker. I haven't seen the brand before. Does cheap condoms give adequate protection?
3) why I am feeling fatigue or low energy to do certain tasks?
you've already been answered. we do not know why you're sore. if the condom was not broken, you're fine. we also do not know why you're tired, but it may just be in your head due to anxiety.

I had a HIV PCR test.

The result is < 36 copies/ml. (ie: <72 IU/ml.

The test method was HIV - 1 RNA Quantitative real time PCR by artus HI Virus - 1 RG RT-PCR Kit validated by Rotor Gene Q.

Could someone please explain what is this. They have not given any reference range or anything.
Can you explain which of the only 3 risks for HIV you think you had? Otherwise you are just making up your own science fiction theory, so it will be impossible to have a logical discussion about that.
The upper limit of linear range detection of the assay is 25x105 copies/ml
I agree with you anxiousnomore. But worried about whether there is a possibility of tear on my barrier which I have used which I didn't notice. Plus I still have severe body pain.

If a condom fails it is a large rip down the seam and it hangs in tatters. Obviously that didn't happen plus it is difficult to take off too. Your pain is in your imagination from fear.
