You are clearly exhibiting paranoia. It is your assumption that she is HIV positive, that information is totally in the grey.
Hypothetically if she was positive, your rectal tear was not deep enough to let the infected blood reach out to your nervous system. Also, there would've not been adequate amount of blood to cause any infection.
I would've been only worried if she had her finger chopped off that moment, it was bleeding profusely and yet she was inserting it in to your anus. Fortunately, that's not been the case. Therefore, there is nothing to worry.
