HIV Rapid Test - 16 weeks?

I had possible exposure in summer of 2015.
Since I have done 3 tests as follows:

1. Alere Determine Rapid Ag/Ab @ 11 weeks
2. Lab Corp ICMA Antibody Only @ 12.5 weeks
3. Alere Determine Rapid Ag/Ab @ 16 weeks

I felt #1 was too early.
#2 happened to be part of regular physical
#3 was to be sure

So my question is...

Was #2 conclusive? If not was #3 conclusive?
Do I need one more test at 6 months?

Also is HIV Rapid test as accurate as lab tests?

Yes 3 months would be conclusive but you can have the DUO conclusive at 28 days.

Suppose condom was not intact.
Are the tests I referenced in #2 and #3 conclusive anyway?

This is more for future reference. If I need to test for potential unprotected events down the line.

Since the condom was intact you had no risk

Protected sex vaginal.
Condom was a bit loose (too big?).

However looked to remain on duration but I am not sure. However did not check that it did not break. Room was dark so just finished and cleaned up without checking.
