was wearing condom during intercourse but later when finished, penis was not on condom

I hope Teak and other experts could help me clarify here.

During a second intercourse with a GRO, I had a condom on (i put it on myself). I intercourse for 15 minutes (didn't cum) and rested and then resume after 10 minutes of rest. After finished cumming, when trying to carefully take out my penis from GRO's private part, i realised the condom was no longer on my penis. Actually i wasn't quite sure as it was quite dark and i couldnt see it was. I was not wearing my glasses, thus it was a blur.

Later after examining the condom, I found that my sperm was trapped in the condom.

According to the GRO, the base of the condom (ring) was still hanging on her vagina. She had to slowly pull it out.
She also mentioned, the condom wasn't stuck-squashed on the end-wall of her vagina as she was pulling it out straight.

All this while, I have been extremely careful but didn't realize this could happen to me.

My question is was the head of my penis covered during and after the intercourse?

I really hope someone with this kind experience could answer this question.

Because it was a 2nd session a few minutes after the 1st session, the sperm residue was not much. I had to clamp 2 fingers on the condom and slide across to the tip to see the accumulation of sperm.

Hi Teak.

i know i did put on the condom well enough to cover my penis's head & shaft and there were some residue of sperms in the condom after.

If we ignored what the GRO said, What is the likelihood that my penis head is exposed during withdrawal eventhough i left some sperm in the condom?
As in the penis head exposed in the vagina wall for a few seconds and then somehow slid back in the condom upon withdrawing.

Hope you could help me clarify.

You need to keep hold of the condom when you withdraw so it doesn't slide off. You did not have a risk.
