Please help ! Interferon and Baraclude

Goodmorning everybody ! I have went through the forum and I realised that forum members have much bigger knowledge and experiance than doctors where I live... I live in Poland. I know about my Hepatilis B infection since 2003. Last year I head big problems with my health I went to hospital and Celiac was diagnosed, I have also made my HBV DNA test it was  409 IU/ml at that time. I don't feel well despite the fact I am on the gluten free diet, that's why I have made more test last month and my HBA DNA was 2816 IU/ml. My Alat, Aspat were about 25 in last years but Alat rised in last year and now is about 35, Ag Hbe is negative, Ag Hbs positive. I have also a little problem with my tarczyca it is to small and doctors suspect Hashimoto but all tarczyca wskaźnik are ok. I also had treatment for Astma but know it is ok and I don't take any drugs anymore. The problem is that my doctor said that I can take part in Polish goverment's treatment program that have been changed this year and I have two options: 1. Baraclude (Entecavir) treatment, 2. 48-weeks Pegasys interferon treatment. My doctor said that I need to make decision by my self about which method I prefer... Could you please help me with this decision ? The problem is if I should choose the Interferon when my health is not to good and on the second hand I am afraid they will cut me out from goverment's program and I will not be able to continue treatment by myself. Another important thing is that my fiance wants to have kids soon and is afraid that when I start treatment it will last many years or lifetime. I will apricate all opinions !

Thank You Very Much !

James

back to my case ;)

I am reading more and more and I have found my old blood test and it seems that in 2005 (2 years after being in hospital) my anibodies anti hbs were: 30 IU. Later in 2007 and 2009: less then 2 IU. Now they are 0 but my Ag Hbs level decrised last year at the same time: HBV DNA went 7 times up. Can somebody tell me if having this HBS anitbodies in past years should change the type of treatment that I will choose ?

the trick is to use antiviral for 1-3 years so that pegintf response is very high on hbsag.any effect on hbsag will reflect on hdv because hdv uses hbsag to infect liver cells

so you have to fight hbsag of hbv to clear hdv

using peginterferon directly has very low response on hbsag, about 7-13% by 48weeks treatment.also keep in mind that hdv infected usually go for 2 years of pegintf this will increase response too

Kozka I will advice you to follow the advice that you have find on our forum, from steff and others, websites from the latest news of Liver conference 2014, as the Steff made me laugh(unfortunately) saying another ignorant doctor. it is very very true, that we may be victims of ignorance of doctors. All you have to do follow resources from here and internet. About the tenofovir you can travel outside around Poland, in Romania,Serbia, Albania and buy a dozens of it.

if tenofovir is not available (extremely weird in europe) you can take entecavir for free and then we will see.pegintf is going to have patent expired and also tenofovir so you may buy it yourself

tdf generic in india is as cheap as 20 to 50usd per month, they will charge more in europe like it happend with lam but not more than 100usd

Thank You Stef !

The only options for free treatment by our goverment is entecavir or pegasys.
The fibroscan result is F0-F1, fats:S0, E (kPa): 4,9; CAP: (dB/m): 215.
Is there any reason I should not take the free treatment from goverment ? Is there possibility it will make more harm that good things ?

entecavir is also not the best antiviral to use and the most expensive.tenofovir is the best choice both for you and healthcare, most potent and cheap

another ignorant doctor to joint the massive group of doctor in need to change job to avoid patients much health damage

the choice is yours but after you know the chances of hbv cure....you need the following to know which is the best treatment for you (you won t deliver or breastfeed babies so drugs have nothing to do with you or babies)

fibroscan, this will tell you liver damage, if no damage you can also make no treatment
hbsag quantity in iu/ml
hbv genotype
hbvdna in iu/ml
hbeag
