Anal bleeding

I have just completed Week 1 (yeah!) on Incivek.  Have had some anal issues since day 2 when I experienced intense diarrhea.  This caused severe irritation but the last 2 days were much better.  This morning there was blood after my bowel movement and maybe even some blood in my stool.  Has anyone else experienced this?  Any suggestions?

Well, somebody finally brought it up.haha! I started to respond privately, and then thot others may be experiencing the same problem. I started my 3rd box of Incivik today. And I found a solution. Blaneol cream. It really makes a protective barrier. When the pharmaceutical rep brought up anal fissures, I raised my hand and asked what brought that on. lol. the room went silent and he simply replied, "Buy Tucks." They didn't help much.
So I now carry this cream in a zip lock bag in my purse w Qtips. And a bag of tucks as well. I can honestly say the bleeding is very very minimal now. Nonexistant most days if I'm faithful to use the cream.
Congrats on finishing ur first week! It's not so bad, when we can minimize the SE. Good luck.

I was on Incivek, too, and I noticed that it did not take long after finishing the Inc. and my anal/rectal area was feeling much better. No more shards of glass bowel movements. No real pain. No burning on movements. However, that was short lived, maybe a couple of weeks, and then I noticed the rectal area was sore and burning again and the hemorrhoids seemed worse. Also itching. I could not figure it out. Why get better and then worse. I had to buy a ton of over the counter meds for my butt(creams, ointments, suppositories, etc.) and I practically went broke on baby wipes. While all of this was going on, I had a worsening generalized rash.It itched like crazy. Eventually, 3 months later, when I finally got on  high enough dose of Hydroxyzine, the rash and the itching subsided and the reactal/anal problem disappeared completely. My thinking is that I had hives and swelling in the rectal /anal  area caused by the same allergic reaction which was causing the generalized rash and itching. I know it was related. as soon as the hydroxyzine was wotking for the generalized rash, the hemorrhoids, the itching, the burning from being excoriated, everything rectal/anal disappeared.

My feeling is this, if I had not gotten that rash and itching, my rectal problems would have cleared up and stayed cleared up. So I am pretty sure that second problem (after finishing Inc.) had nothing to do with Inc.

The reason I mention all of this is because it is quite possible that the problem you have now is not from the Inc being in your body at this time, but rather it could be from some other complication or problem. It is possible you developed internal  fissures and that they are still causing a problem. The fisures could cause pain. Or, maybe you have developed some sort of IBS as a complication of the Inc or the other meds.

If I were you, I would talk to my treating doc about this. In addition, I would see a proctologist or someone who specializes in problems with the colon. Not all GI docs are proficient in colon problems. Some specialize in colon problems. At least then you wouyld have an expert trying to figure out what the problem is.

I really hope this problem clears up for you. I know how miserable it can make a person.

Just a followup...

I'm not off the Incivek and after some trial and error at managing the rhoids and burnging we concluded that the best way to manage it was to take antidiarrhea medicine, and apply Hydrocortisone cream (I found CVS brand - Intensive Treatment -- blue tube, red lettering to work best).

This managing kept me from stopping treatment and Thank God I was eligible for STD, because I would not have been able to work because it was so much pain.  Even while managing it successfully.

My latest concern is I have now been off Incivek for over 4 weeks, and I was under the impression all the sysmptoms would go away after a few weeks, and they have not.  It's almost like I have some type of IBS, now, since after every movement I experience extreme discomfort in my anorectal area... during and afterwards for most of the day.  

Has anyone else made it this far and experiencing these same issues/concerns?

On the plus side, I am still undetectable going into my 16th week.

Tx: Incivek, Pegasys-Interferon and Ribavarin
Period:  2/11 3rd injection (Start of 3rd week)

I was a previous responder and completed the original treatment of using Inteferon and Ribavarin.  Viral count returned once treatment had stopped, though wasn't detectable at all, the entire treatment, after 4 weeks.  That is what prompted me to start with the Incivek ASAP.   Other than the flu-like symptoms I originally experienced the first week or so, I had no side affects other than general fatigue.

This time around with Incivek, Rhoids set in within 72 hours and escalated to rectal bleeding, followed by an ER visit at the direction of my physician.  In total it lasted about 9-10 days and (knock on wood), has gotten smoother the past few days.  Light rectal bleeding though movements can be very uncomfortable.  My doctor keeps telling me "the rash" was suppose to be the big thing with this Tx, but they are seeing very little of the rashes, and a significant amount of rhoids.  He also kindly told me they were basically unprepared to manage these symptoms really and caught off guard.  

Nonetheless, we have been working on trying to manage these symptoms.  Other than a very mild case of itching, I haven't experienced much itching or rash.  It's all about the fire-in-the-hole symptoms for me.  At first we tried suppositories, 2X day, but these just made me feel like I needed to go more.  I have also incorporated a warm-hot bath with lots of baking soda and have been using Triple-Paste(diaper rash cream), for my bottom area.  After my morning movements, I get right in the tub, soak and circulate the whirlpool.  I clean the area and pat it dry and apply some cream every other day.  

Generally I try to manage my movements, while I am taking the required fats with every dose I do try to manage my meals to keep me from going as much or too heavy, and shoot for morning and nights.  I have lost at least 10 pounds at this point and some anemia seems to be setting in.  Unfortunately, the biggest problem I have right now is the intense pressure in the rectal area after a movement or two.  Someone refer to this earlier as "the wiggles". It's not the buring sensation associated with movements, usually I use tissue to soak up any leakage around my rectal area after movements.  I have to repeat that technique 2-4 times after a movement.  I suppose that is what everyone else is using tucks for?    This has seem to help heal the skin around the area, and lessened the duration of any burning and discomfort after a movement.  However "the wiggles" for me is when after a couple of recent movements, I clean the area, but it feels swollen, and significant amouts of pressure that is so uncomfortable, all I can do is rock back / forth on all four, squat or do some stretching activites.  Either way, I wish I could just control this side affect at this point.

I hope this helps with some of you and reading all your post have been helpful to me tonight (this morning), as I can only rock back and forth right now trying to figure out what to do next.

I thought I was going crazy with these fissures.  I have been to surgeons, corlerectal specialist, the ER...Treatments have ranged from: creams, ointments, suppositories, botox injections, and stiches for the fissure that just woud stop bleeding. Someone please throw me a "life line".

Just a update !!!
I am sold on the Fat intake, I got up today day 4 and all rectal pain is gone, only thing I have done is eat FAT and it went away.
I am guessing that rectal problems are caused by to Incevik not being able to leave the body.
Give it a try cannot hurt.

Wow great tips. I'm starting week 4 with Incivek and this is definitely an issue for me. What is baneol cream ? Is it available over the counter? I've been using charmin flushable whips, tucks and destine, which is for diaper rash. I figured I needed to protect the delicate tissues down there and it all has helped.

I'm so happy someone brought this up, too!  I can tell all my friends about the different side effects I'm experiencing, but I stop short of this one -- after all, who really wants to hear about 'roids, anal bleeding and God-Awful itching???  I don't want my friends to have THAT visual of me!  LOL

Seriously, tho, it's become such that I dread going to the bathroom, and with all the water I'm drinking, and the fat I'm ingesting, I'm a frequent bathroom visitor lately... :(

I have my supply of Tucks, and Prep H, and even Boudreaux Butt Cream, which helps a bit, but nothing, and I mean, nothing, completely relieves it.  It's the most uncomfortable of my side effects thus far.  

On the bright side, I'm 6 weeks in on the INC, so I'm halfway to the end of this dreaded medication.  So far, the Riba and Interferon have been a breeze compared to the INC -- and, trust me, they've been kicking my *** mighty fine on their own, thank you...

Anyway, I'm hanging in there, like all of us -- Thank goodness for this Forum -- I don't know how I would be doing without it... I don't feel so alone, ya know?

I am already eating a lot of fat but maybe I will try to eat more. I did have a major problem with rectal pain (internal) (felt like I was passing shards of glass) a few days after starting the Incivek. It lasted about 6 days and then disappeared. It came back 2 days ago, was worse yesterday, and now today seems to be almost gone. I am hoping the no pain days outnymber the shards of glass days, LOL. I will try anything so I will be eating ecen more fat, if that is possible. I am also using 3% lidocaine/0.5% hydrocortizone cream and hemorrhoid cream too.

I went through Incevik finished 3 weeks ago UND . But I neaver had this problem until 9 days after finishing ?
Called Incevik Nurse Hot Line and they asked if I had cut way back on the FAT!!  I said yes and they said start eating more for awhile ,That Incevik is FAT Soluble and it comes out through Small Intestine!
Well I upped it and is much better 3 days later,So I think anyone having any recital problems should up there FAT intake!!!!

I got a script todayfor hydrocortisone suppositories...oh joy of joy.  I will let you know if it helps.  Mr. Bidet also sounds like a something to try.

does anyone have any experience with an otc called Nupercainal ? found it on an internet search and was wondering if it was any better than preparation H.

I used proctofoam during my tx. It's rx, not otc.  It helped, but here I am hoping the balneol will help better for my daughter when she starts. The lidocaine ointment sounds promising also.

Woudn't it be nice if we could come up with an equivalent to Magic Mouthwash for the other end of the alimentary canal?

Just saying.....any pharmacists out there????

Something I have been using for several years (after a visit to Thailand in 2008, where they were everywhere)  is a Mr. Bidet.  It is basically a squirt hose (google it, amazon sells them for $40)  it connects to your toilet easy as pie and reduces/eliminates the need for wet wipes and toilet paper.. which reduces irritation.  Plus you just feel so cool and clean afterwards!  :)

Ask your doctor for some lidocaine (sp?) ointment.  It helps a lot.

I'm in the same boat. I started treatment on Thursday and by Sunday it all started. Even worse I was at work and could barely walk or sit. The tucks burned like HELL and the blaneol cream is ok. Now um trying the prep h. I hope this works. I hope you fund the right combo.

The Balneol cream suggested by crossroadsec has truly helped.  I've taken all the other suggestions to heart and have implemented many of them.  The balneol actually contains mineral oil and provides almost instant relief.  By the way I got robbed at the CVS where I paid $18 and later looked online at google shopping and found it for $6.

Thanks also to Isobella for the dose of humor.  I strongly believe that a good laugh is still the best medicine.
