Ribovirin in the fridge

I accidentally put my ribovirin in the fridge x 5 days before I started treatment. It came on ice and the pharmacy that sent it stuck some label over the "how to store instructions" on the box. I am worried that the medicince is not working because of the wrong storage x 5 days. Any advice?

I can't tell you how many times to post or what to post. This is a public forum so you can do pretty much whatever you want. But I do have to warn you that MedHelp monitors these forums and takes it very serious.
Not that it should matter to you but I like some of your posts, just think you take it to far many times.
Take care

I hate to come across like a d*ckhead, but I agree with Copyman. I'm sure you're a cool dude, Rocker, but you come across like someone who wants to share his Motorhead albums and share a few beer-laced humor and tacky jokes versus someone who wants to seriously discuss this issue and life revolving around it.

That's just honest feedback, man. Do with it as you will. I wish you no ill design.

how many times are you going to post in a thread? I thought you said you were going to tone it down.  Guess not. I guess your attitude has changed once again and now it is "if you don't like it ignore my posts". Quite honestly it is hard to ignore 10+ posts in a thread! and especially when it is WRONG information. That post about taking riba without food can be harmful to someone. This forum is to help people. If you are not going to do that then you have to go!  Come on dude there are people here treating that you are bothering and I'm one of them! get it together.

53 grams of fat.....roar!!!

we better start a class action to get our Hover-Rounds paid for in that case!! We'll all be needing them.  Just wheelbarrow me to the docs and home!!

So basically, eat a big fat juicy tenderloin with your Riba....??? What about heart issues, weight, or fat helping virons to survive, etc.
Maybe this might be wisdom to get UND more quickly, but once you are UND and your blood level of Riba is high getting the max absorption may not be as critical as just keeping some going in. I mean provided you are staying UND it sounds like overkill

to ignore the potential downside like oxidative stress, peroxidation, plaque formation etc and just drink a bottle of fat everyday. Especially since the viron survives best given an opportunity to devolop a strong lipid shell, it makes sense to consider more than just absorption rates at some point does it not?

Effects of Food and Antacid on Bioavailability

Two studies have been carried out to assess the effect of food (ie, a high-fat breakfast) on the bioavailability of single oral doses of ribavirin (Table 2). An initial study by ICN Pharmaceuticals, Inc. (Costa Mesa, Calif)[19] showed minor effects on bioavailability, but a 33% increase in Tmax. A subsequent study by Schering-Plough Corporation (Kenilworth, NJ)[6] demonstrated more substantial increases in bioavailability: food increased the AUC and Cmax by 70% relative to the fasted population, and Tmax was more than doubled.

Although both studies consistently showed that food slowed the absorption of ribavirin, the extent to which food affected ribavirin bioavailability differed widely between the studies. It is also unknown whether any food effect might be altered by the type of meal consumed (eg, high versus low fat), if a food effect would still be evident upon multiple dosing of ribavirin, and, finally, what clinical implications the food effect might have. It should be noted that in the pivotal Schering-Plough clinical efficacy studies, ribavirin was administered without regard to food. Because of the uncertainty about the effects of food, it may be prudent for patients to take ribavirin with food; if toxicity (anemia) occurs, this can be managed by ribavirin dose reduction.

Coadministration with an antacid containing magnesium, aluminum, and simethicone (Mylanta®, Johnson & Johnson · Merck, Fort Washington, Pa) reduced the bioavailability of ribavirin (mean AUC reduced by 14%; Cmax reduced by 3%).[6] This change is unlikely to be of clinical relevance.

Ribavirin is absorbed from the GI tract probably by nucleoside transporters. Absorption is about 45%, and this is modestly increased (to about 75%) by a fatty meal. Once in the plasma, ribavirin is transported through the cell membrane also by nucleoside transporters.

Why would you even post information from a site like that as the advice you're passing on to someone else?  You need to take a little responsibility for the information you're passing along and the source you're taking it from or your info will start to be taken with a grain of salt.

Ronnie, you can’t be serious… you’re quoting from an offshore Viagra site! I’m not sure they’re very authoritative:

http://www.medsupport.org/ribavarin.htm

“…This medicine may be taken on an empty stomach or with food. You should always take it on an empty stomach or always take it with food. Taking this medicine sometimes with food or sometimes on an empty stomach may decrease the effectiveness and/or increase your risk of side effects. It may be best to take this medicine with food (more medicine might be absorbed into your bloodstream)….”

LOL!!!

From Schering-Ploughs PEG-Intron prescribing info:

http://www.spfiles.com/pipeg-intron.pdf

Effect of food on absorption of ribavirin:

Both AUC and C-MAX increased by seventy percent when Rebetol Capsules were administered with a high fat meal: (841 kcal, 53.8 g fat, 31.6 g protein, and 57.4 g carbohydrate) in a single-dose pharmacokinetic study.

"You should always take it on an empty stomach or always take it with food. Taking this medicine sometimes with food or sometimes on an empty stomach may decrease the effectiveness and/or increase your risk of side effects."

What information do you use to support this, that you need to pick one and stick with it to impact effectiveness and side effects?  Can you please quote this info?  I'd be interested to read this.

From what I'm aware, studies indicate that taking riba with food doesn't increase it's absorption factor but taking riba with a high-fat meal DOES increase absorption factor by approximately 46%.  So neither taking it on an empty stomach nor taking it with a meal has an impact on effectiveness.  It probably hits your stomach better to take it with a meal but to take it with a high-fat meal is the MOST effective by a considerable margin.

"The component of the final population pharmacokinetic model that describes the absorption phase is complex. Although a standard meal did not affect ribavirin bioavailability (F1), administration of ribavirin with a high-fat meal increased bioavailability by 46% relative to the fasting state. A high-fat meal prolonged the duration of the zero-order input part of the absorption model, with D1 increasing from 0.498 h (fasting and standard meal) to 0.740 h. The type of meal also influenced the first-order input part of the absorption model (Table 1)."

I copied the following from Copegasys instructions on the net:

Store the packages, vials, or pre-filled syringes in the refrigerator, protect them from light, and keep them out of the reach of children. Do not freeze, shake, or use past the expiry date. Ribavirin tablets can be stored at room temperature or in the refrigerator. Keep them out of the reach of children.

It's pretty funny that they say store it at room temp (77 degrees F)...heck, it's wintertime and my house is never anywhere close to 77 degrees...more like 62 during the day and 55 at night.  :)
