They have reared their ugly head... Any suggestions?? We are giving him plenty of fat at pill time... I'm thinking he needs to do it 30 mins prior but dr said within 30 mins. There seems to be blood internally, I'm thinking hemmoroids and he's pushing hard stool... Sorry so detailed, but need help!!!

Balneol cream brought a lot of relief for me. It's cheap and doesn't cause burning like tucks. (for me anyway) Also, remind him not to sit on the throne for too long. There is an end to it! Karen:)

You've got some good advice above, I'll just chime in and say that 15 g of water soluble fiber every night helped my husband immensely, and within a few days.  Balneol cream also worked quite well.

I just posted in Quayle's thread and I will copy and paste the same post here. It is written to Quayle but just substitute "your husband" every time I said "you.":

It is a common complaint, but that does not mean you should have to tolerate it. Your Hep doctor could help you with this. (And, I have found that they seem to have no clue, which is really a shame considering how common it is.)

Be sure you are eating at least 20 grams of fat with a meal within 30 minutes PRIOR to taking the Incivek.

Drink plenty of water. My doctor said 1 gallon a day.

There are a few different anal/rectal side effects and I found that each one requires a somewhat different approach/treatment.

Diarrhea/loose stools: Diarrhea and/or loose stools will aggravate every other anal/rectal problem you may have so keep the stools firm (not like rocks, but quite firm). This is gross, but they should be firm enough to be able to throw them or to be able to mold them into an object (like clay) (and no, I did not throw them or mold them, just an example). I used Loperimide (Immodium) as needed. I did not take it every day but at the very first inkling of a soft or loose stool (cramping) I took 1 Loperimide tab. A person can take more, but I started with 1 so as to take only what I needed. Some use commercial fiber but I stayed away from commercial fiber (metamucil, fiber pills, etc.) so as not to interfere with the absorption of the Incivek. I read some place that the commercial fiber can interfere with absorption, so I never used it.

Crapping shards of glass or crapping razor blades: (This is probably caused from internal fissures created inside the rectum by the burning diarrhea.) Not everyone gets this problem. In fact, most do not. However, if you get it, you will know. It is literally like crapping shards of glass. The pain is excruciating, grit your teeth, scream and swear, hang onto the vanity type of pain, break out into a cold sweat type of pain. So you will know if you get it. The solution, though, is very simple ..... keep the stools quite firm. I know that sounds too simple, but I can tell you it works. I thought I was going to die until I figured this out. (And my Hep team was absolutely no help.) I had it 3 times, the first time was horrible because I did not know what to do. Consequently, it lasted about 2-3 days. Not constant pain, but sheer agony every time I had to have a bowel movement. Then I discovered that if the stools were firm, I had no pain, absolutely no pain. Two other times it started but I firmed up those stools and it went completely away. So, the key here, is keep those stools firm.

Burning butt: Keep the stools firm because if the stools are loose they will irritate the anal opening as well as the skin it touches on the outside. In addition, use Huggies baby wipes with aloe to clean the area. (Do not use a generic, they will sting.) You have to clean the area so it is spotless. One tiny little piece of stool will irritate the skin and cause you major grief. You can also shower after every stool but if you are going a lot, that can turn into a lot of showers per day. Also, you can use A&D ointment, Baneal cream and wash, Butt Balm, Desitin or some other soothing agent to coat the area and protect it if the area is excoriated or raw. Sitz baths help.

Hemorrhoids: You can try Prep. H cream and suppositories. They may help. I used them every night before bed. I think they might have helped a little, but not a lot. I think Annusol HC Supp. would work better because they contain  hydrocortisone. Several people on the forum used Annusol HC supp. and they helped a lot. If I had it to do over, I would demand the Annusol HC supp. They are prescription. Here is what drugs.com says about them: "Anusol-HC suppositories is a topical corticosteroid. It works by depressing the formation, release, and activity of different cells and chemicals that cause swelling, redness, and itching."
I also tried Prep. cream but it stung and seemed to do no good and it irritated the flaming red butt I had. I did not use it.
I never got rid of the hemorrhoids until I was off Incivek so it is a matter of managing them during that time.

I hope I did not forget something. If I did, I will think of it or someone else will.

If the hemorrhoids are the main problem, then, if I was you, I would ask for a prescription for the Annusol supp. I think they will work the best for the hemorrhoids. If you cannot get a hold of your hep doc then you could ask your primary doc.

Best of luck.

A guy who works with me was complaining constantly about his hemorrhoids. I believe that he may also have had an anal fissure.

Anyway he tried everything from over the counter meds to prescription meds and nothing helped. I researched this issue and stumbled on a discussion of sufferers and the one thing everyone who tried it agreed that it worked was;

DESITIN!!!!!!!!!!!!!!!!!!!!  Yeah, the stuff for diaper rash.

I know it sounds crazy but it really worked for him.

Google "Desitin Hemorrhoids" and you'll be surprised.

Good luck,
Mike

This is a very difficult side effect that many have had with Incivek.  I am posting a link to my journals where I listed some of the ideas that many on this forum suggested for this side effect.
