how long on harvoni

Was on Incivic for 48 weeks with riba ad interferon. Showed undetected for 9 months but at 6 months after treatment, showing detected. Docs have suggested Harvoni now, but at 12 weeks with riba. Is this typical of should I be doing 24 weeks?

Hi

I am just finishing 24 heeks of Harvoni and we added Riba later so 15 weeks of Riba total. I am done tomorrow!

I am a multiple null responder to interferon based tx never did the incivek because I had cirrhosis by then and the stats said I had about a 14% chance of it working.

I was dx with cirrhosis in 2008 and relapsed last year after 12 weeks of Sovaldi Olysio.

I saw a you tube video of a round table discussion of some experts discussing their thoughts just after last years AASLD conference discussing many topics amoung which was would the recommend ribavirin for a patient like me and they agreed yes they would to give a patientr like this the best chance of a cure. So I discussed this with my doctor and shee agreed it would be a good idea to add the riba.

But in general if your not as a hard luck case as me you could go with 24 weeks of Harvoni or 12 weeks of Harvoni with ribavirin they are equally effective.

Treatment length of time depends on your liver fibrosis stage and prior treatment results.

"Daily fixed-dose combination of ledipasvir (90 mg)/sofosbuvir (400 mg) for 12 weeks is recommended for patients with HCV genotype 1a infection who do not have cirrhosis, in whom prior PEG-IFN and RBV treatment has failed."

Rating: Class I, Level A

(The same treatment is recommended for those with Genoype 1b who do not have Cirrhosis.)


"Recommended regimens for patients with HCV genotype 1a or 1b infection who have compensated cirrhosis, in whom prior PEG-IFN and RBV treatment has failed."

"Daily fixed-dose combination of ledipasvir (90 mg)/sofosbuvir (400 mg) for 24 weeks is recommended for patients with HCV genotype 1a or 1b infection who have compensated cirrhosis, in whom prior PEG-IFN and RBV treatment has failed."

Rating: Class I, Level A

"Daily fixed-dose combination of ledipasvir (90 mg)/sofosbuvir (400 mg) plus weight-based RBV (1000 mg [75 kg]) for 12 weeks is recommended for patients with HCV genotype 1a or 1b infection who have compensated cirrhosis, in whom prior PEG-IFN and RBV treatment has failed."

Rating: Class I, Level B

I do know that several forum members with Genotype 1 and Cirrhosis are treating for 24 weeks with Harvoni and Ribavirin. The SVR rate is slightly higher with the addition of Ribavirin (to Harvoni treatment) for 24 weeks (as opposed to 12 weeks). I will try to find the link to that data.

If you are cirrhotic you should be doing 24 weeks.  If not then you will do 12.  I hope it works this time for you.
