Based on the question detail, what's causing my discomfort?

Last month I visited a massage spa. The role play got out of hand. The attendant guided my penis between her tights. There was no penetration but genital rubbing and her vagina lips may have touch my penile head. I sense danger and my hard-on quickly went limbo. 3 weeks later I notice tingling, burning and itchy sensation all around my genital area. It persisted. At 4 weeks I notice what looks like an ulcer inside my penile urethra. (I actually have pictures, but I don't know how to post it here). I quickly went to the clinic. They took a swab of my urethra. Test result came back negative for herpes. I have confronted the spa attendant and she said she been vaccinated against herpes. There is no vaccine. There is a therapeutic vaccine. I'm freaking out. It's now well over a month, and I have this constant discomfort on the head of my penis. It feels like there's something there like something is poking me. Sometimes I feel a burning sensation there and sometimes I feel like pins and needle are poking. Can this be herpes? Can herpes cause constant discomfort every day?

It could be many things but I again doubt any std is the cause. You might want to see a urologist.

What you describe does not sound like hsv at all. It would be very rare for symptoms of the infection to come on three weeks later. I doubt any of this is from any std.
