Reliability of Igm typing

I had both HSV Igg and Igm testing as well as viral culture on genital scab.

HSV 1 Igg 5.8
HSV 2 Igg .12

HSV 1 Igm .28
HSV 2 Igm 3.2

Viral culture was negative. In regards to lesion, doctor originally thought it was staff infection from abrasion/irritation. My questions are 1) how likely is It that the Igm testing was inaccurate in terms of typing HSV. 2) how likely is it that I really do have genital  HSV 1. 3) could the mild to know symptoms( slight tingling but not very often) are because HSV 1 in genitals is less severe? 4) should I do another follow up Igg test?

It is still much more likely that the infection is oral given the reasons I mentioned above.

Sorry for the many posts... The scab culture I had was during the second round of testing six weeks after and that culture was negative. I had had the scan for about a week.

The doctor thought he scan was due to irrational from cortisone and Neosporin. I was using that because of irritation from condom during same exposure.. But still had in protected oral sex unfortunately

Thank you again. So just a thought on your last comments... Even though I tested negative on hsv 1 and 2 Igg two days after exposure of unprotected oral sex, then roughly 6 weeks later I retested because of a couple of red blisters on head of penis that scabbbed over (never any pain at all, just slight tingling in penis) and those results we're  what I first posted?

It is not known, probably of the order of about 2% over a year of sex.

There is no reason to suspect you have a genital infection however.

Are you aware of the transmission rate/ likely hood of genital hsv1 during genital to genital contact? This is for future reference on my part.

Testing for HSV1 misses about 1 in 10 infections, so not that uncommon. I admit I do not know how this works but the different lab thing would possibly mean a different manufacture's kit with different antigen production and so on.

It is more that the sore appeared over 2 months after the exposure. HSV1 is much more likely to cause problems from day 2-10 onwards.

So yes overall I think what I believe about your situation remains the most likely explanation.

And yes the first and second test were different doctors and I'm almost positives different labs. Second test was quest diagnostics.

I tend to agree with you but am curious as to how I would have a false negative on the first Igg test for HSV 1, I've heard this Is rare. I do in fact see a lot of references about false Igm test in terms of typing between the two, which might explain the positive HSV 2 Igm on the the second test.. The blister on the head of my penis is healing well never had any pain, especially anything that I have read about how Hsv symptoms feel. Plus the viral culture swab of the scab during my second test was negative.

Does this again confirm your thoughts?

The most likely scenario is this:
* You have had an oral HSV1 infection since your youth
* The first IgG test did not pick up the infection (was it a different manufacturers kit to the second one)?)
* You do not have HSV2.

Why? A first infection with HSV1 doesn't show up as a vague red dot on the head of your penis two months after protected oral sex. There was no practical exposure to HSV2. IgM has no value when you have a type already and is no value 8 weeks after possible exposure.

There is no reason to suspect genital herpes.

The first test results that were Hsv 1 and 2 Igg were .12 and .20 respectively.
Again the first test was two days after exposure and second test was two months after exposure to unprotected oral sex.

I had HSV 1 and 2 Igg test done two days after exposure. All results were negative. Then two moths after exposure, I went into get retested at a different clinic and I was given both Igg and Igm test for both. HSV 1 and 2. This results are posted above, indicating positive for HSV 1 in Igg and positive in HSV 2 for IGm. The reason I went in for a second test is because red spot developed on my head of penis and then a few Kate's appeared and scanned right over. No pain or burning while urination. No sensitivity just I assignable feel a little tingling prolly cause I'm looking at it all the time. My original question was due to all the reading I've done on inaccurate typing for. IGm test. I'm going to go back and get rest test for Igg." In a couple weeks. In you opinion do you think the second testing was off and wrong in typing me with Hsv 2 in the Igm test?

Back up a step, what was the testing you had done ten days after exposure? Was it definitely IgG and what were the results?

Once you have one type of HSV, then IgM is useless for a number of reasons in determining a further infection.

If the exposure was oral, then HSV2 was not a practical risk in the first instance.

When in relation to the exposure and where did the symptoms appear?

I'm just looking for clarification on recommendations.  Again two days after exposure of in protected oral sex I had HSV Igg test come back negative. About two months after that I retested and received the results above as listed in the opening post. Just looking I see if I should retest and if you might believe it ign was inaccurate , even thought you stated you don't use them

Again I'm starting to be 100% sure i do not have HSV 2 and that the IGM testing was inaccurate when it said positive for hsv2

Thank you again for your quick reply. This testing was done almost two months after possible exposure. I had HSV Igg tests den two days after exposure and everything was negative. Should I still follow up with another Igg test in a month or so?

Your test is conclusive if you tested at least 3 months post any exposure. Blood testing doesnt say where you have it but its most likely oral from childhood.

Also any thoughts on the severity of my symptoms in relation to hsv1 genitally?

Hi, we dont  use igm only igg as from you test your positive for hsv1 only.
