What could this testicular discomfort be?

I have a 3mm varicocele, but my Urologist doesn't believe it's what's causing my left-sided testicular discomfort with occasional radiation into lower left quadrant of abdomen. I am Chlamydia, Gonorrhea, and HIV negative. Testicular and pelvic ultrasound are negative for a mass, hydrocele, or spermatocele in the testicle and negative for an inguinal hernia or illio-inguinal hernia in pelvis. I am on max NSAID (naproxen) daily and if I miss a dose, the discomfort is quite bad.

History: I am now sexually active with one partner monogamously, but have had multiple sex partners within the last year. Eight months ago I had a clear discharge that resolved without antibiotic, but have since been on two rounds of Doxycycline to address a possible STI. No lesion, purulent discharge, or urinary discomfort has ever been experienced. I am a weight lifter and I make every effort to never lift with a full bladder so as to discourage any reflux epididymitis.
