What can I do about groin pain and an ache in my balls?

I pulled my groin muscle while moving boxes a few years back and experienced a fever a few hours later and the following days had epididymitis. They think it may have been caused from bacteria getting from the gut to the balls or from a UTI. No STDs present.
Since then I have noticed reduced libido, possibly due to lower testosterone, but still capable of getting erections. I often still feel pain going from groin, thru my stomach and up to my heart. A week ago I seemed to have pulled it again during sex and now I can't get an erection and my balls sometimes ache. I don't have insurance. What can I do? Could the testes be damaged? How can I get them checked?
