Painful Rough Skin Penis Glands

A year back I went to a local sexual health clinic after panicking a little about pain on my penis glands. It's like a scratchy, painful pain when I masturbate, and more tame but still there other times. I can see raised and scattered bumps on my glands, and if I touch them more directly they do hurt more. Another way to describe it is kind of like chapped pain. And it Also feels cold/kinda numb. I've only had sex w/ two partners and clean well.
During that time they prescribed me Clotrimazole and I tested negative for STDs on a panel. After using it like they said for two weeks I was still struggling and the pain was increasing. I went back 2 months later and they doctors said it could've been related to a reaction to soap/detergent, chaffing, and so it didn't seem to spur from that but - I got new soaps + detergent and used them consistently and got softer underwear. But I don't feel like anything has improved, and it takes me longer to come.

Note: I can take a picture of what I'm talking about I just don't know how to on here.
