We have talked a lot about a our rights to copies of our medical records.  I found this great site that has info and Letters that can be download free of charge.  Some examples, requesting copies, asking for a second opinion, not a covered benefit and so on.   Just go to patients are powerful.  It is an org not com.  This site also offers free legal advice in areas of malpractice, wrong dx, etc.

Thanks for the link! I request my records from my neuro in Sept... and I still don't have them... I'm feeling a bit more empowered now.
