There are many possible causes including whiplash neck injury in a road traffic accident which could also cause spinal fluid leakage and headache and dizziness. This could be due to cervical cord compression due to degenerated or prolapsed intervertebral discs in the cervical spine causing central canal stenosis. These can be visualized on an MRI cervical spine. Consult a neurologist for a detailed and systematic examination. Therapy would depend on the diagnosis. Anxiety is one of the causes of dizziness indeed.
The Mayo Clinic's web site lists anxiety as one of the potential causes of dizziness. There are a lot of new studies that support this.
If your doctor is not familiar with this I can send you some links to give him.
I suggest you look into possible causes of your anxiety and other issues.
Have you looked in possible nutritional deficiencies? A thorough investigation, including B vitamins, minerals( blood tests do not show all mineral deficiencies- like Magnesium) and EFA levels.
Omega-3 is an important one to check also.
As well, food intolerance,reactions and allergies specially "silent" ones
