The things I am describing are not necessarily shoulder either. They are nerve conditions, and you describe a nerve condition.

That sounds very strongly like a nerve issue.
It can be impingement syndrome, it could be thoracic outlet syndrome, it could be nerve impingement, or cervical related.
There are multiple shoulder exams to narrow down the problem.
He needs to see an orthopedic Dr. that specializes in shoulders and can recognize nerve issues and cervical related issues.

I'll have to ask him what the pain feels like if it was nerve tissue is there anything he could do about it? Now and again at the top under his arm you can see something twitching like a pulse it doesnt hurt when that happens but he cant control it

Do the tops make the pain seem tingling or burning?
Positional and the tops make it sound like a nerve issue.

He has pain at the top of his arms if he wears tight tops or if his arms go in certain positions

its the tendon in his elbow going up to his tricep he never found out how serious the tear was as he left it 2 years before going to have his scans because he had back problems which he had to use health insurance for first..he was weight lifting and he over did it and felt a pop and the pain was to bad for him to carry on.

You don't say what tendons and what degree of tears. You don't say what was done, and what type of pain he has.
The MRI could be fine, but that doesn't mean he doesn't have scar tissue, or pain from compensation.
