Does lot's a pain usually mean malignancy??

Hi. I have recently been diagnosed with a 4cm complex mass in the left adnexa and my left ovary is 7 times the size of the right ovary which is normal. I have a question about pain. I had an ultrasound in October 2009 and there was not cyst or masses found the and my left ovary was fine. I was in pain at that point and had been for a long time, but about December 2009 is when I noticed a considerable increase in my pain. I have really intense lower back pain, it's also right down in the tail bone and radiates down my legs, pelvic pain and cramping, stabbing pain in the left and right sides of the lower adbo, sometimes sharp pains in the upper abdo as well, hard, swollen stomach, frequent urination, nausea, dizziness and fatigue. I had all of these symptoms on and off for a long time but they began increasing last December and by now they are excruciation and the frequent urination and urination urgency and driving me a bit bonkers :) Pain killers are not doing much even had oxycontin during by periods and they didn't even fully relieve the pain, I can't even imagine what my period would have been like with out them! From anyone's experience does having more pain and symptoms and/or a steady increase in pain and symptoms mean there is more chance of malignancy? I know that it is called 'the silent killer' and that usually is not found in later stages because that is when symptoms present and I have read that cysts usually have few if not any symptoms but ovarian cancer does. I hope this makes sense, Thank you. :)

Yes, I feel a bit better knowing more about what is going on. I'm pretty sure I will have surgery to remove at least the Endometrioma, if the corpus luteum is still there and'or bigger I assume they will removes that as well. I can't have a laparoscopy because of my scar tissue from previous surgery's so they will have to do a laparotomy.  I will post and let you know after my Gyno appointment next Tuesday :)

I'm glad you got some answers.  Now you can go into your appointment with your gynecologist informed on what is going on.  That is always helpful.  Do you think that surgery will be scheduled?  Please let me know what you find out.  Good luck!!

My doctor rang me this afternoon as he had just received my ultrasound scans from today and wanted to check that I had my Gyno appointment soon. He also wanted to see how I was pain wise and if I needed anything for it, God bless him!. The ultrasound tech said he thought the cyst on the left maybe inside the ovary but the doctor thinks it maybe on top of the ovary. The doctor also said he is pretty certain it is an Endometrioma. The cyst on the right ovary that was found today is a corpus luteum and he thinks that it may just resolve itself, as it was not there 4 weeks ago. The first cyst I had on my left ovary was a corpus luteum and I had to have it removed as it didn't go away on it's own. As this one had only just shown up, is only small and I have about a week unitl my period is due, we will just have to wait and see.

Had another ultrasound today,both external and internal. The ultrasound tech said he thinks the mass is actually inside my left ovary, it is still complex and has grown almost a cm bigger than 4 weeks ago. It is also an irregular shape. I also have a cyst on the right ovary that may be just a functional corpus luteum, it was not there on the last ultrasound. My stomach is again very swollen and hard today and I am urinating every 5 minutes, the pressure is very painful. I am walking like a duck! LOL It is very much like being 9 months pregnant and just waiting for the baby to come out because you are just over it! I got the scans to take to the Gyno next weeks, so it's interesting to be able to look at them. Only 7 sleeps until my Gyno appointment, let's hope it goes quick! My pain gets worse everyday, but I thin the worst thing is not knowing what's going on. I think once I have seen the Gyno and know what the course of action is, I will feel a bit better. Mentally anyway :)

Please let us know what you find out.  I am glad that your doctor is being an advocate for you (Sorry I guess I read your post wrong and thought your doctor didn't want to do anything about your cyst).  It sounds like you are going to be in great hands.  When you have a complex cyst, there is always room for concern.  It is good that he doesn't think a CT is in order.  Good luck tomorrow!!

I have my gyno appointment tomorrow April 27th.  I will let you know what he says because I have got a ton of questions for him this time.

Hi, sorry, your post wasn't there when I wrote mine :) My doctor said I could wait 4-6 weeks and have another look or see a Gyno. I opted for seeing a Gyno as I have been through this before and know that being complex can be more reason for concern. I wasn't happy with the Gyno I had been seeing so he wrote a referral for a new one but I couldn't get into see her for 4 months so I rang around and found a really good one that I will see next week. I have still had to wait 4 weeks so suggested to the doctor that I get another ultrasound before I see the Gyno, so he has recent results, and he wanted me to have another one around then anyway. My doctor did say that he thought I should go to major city hospital so I can be referred to other doctors after the Gyno if need be and that I should be a priority to see the Gyno. As at the time I only had 3 weeks until my Gyno appointment he said that it is ok, but he wouldn't want me to wait any longer than 3 weeks. Also, as the Gyno appointment is at a hospital, he said I could still see other doctors and be admitted straight away if need be. He said he didn't think there was enough concern for a CT scan straight away as this can do more harm than good, but if he really thought I needed one he would order one. So I guess he is saying that there is cause for concern, that I should see a Gyno as soon as possible, but from the ultrasound it doesn't look like I need a CT scan or other tests immediately. I guess I will find out more tomorrow when they take another look.

HI, as I have said in my post a complex mass has been found but the doctor is unsure of exactly what it is, most likely a cyst but do not know yet what sort. I am having another ultrasound tomorrow and then see the Gyno next week. Sorry if my questions confused some people. What I am asking is doe having more pain mean there is more chance of malignancy. From everything I have read usually ovarian cancer is said to not cause much if any pain in the early stages. As my pain and symptoms are getting worse by the days, I wonder if this can point to the mass being malignant. I have been told and read that the mass being in the adnexa and not actually on or in the ovary or on or in the fallopian tube and the fact that it is complex, make a higher chance of malignancy.

Having bad pain or in increase in pain does NOT mean cancer.  Any cyst, simple or complex, bit or small, can cause pain. All of the symptoms you described are normal for having a cyst.  What have you and your gynecologist decided to do about your cyst?  In my opinion, if it is complex it needs to come out, only because there is solid and fluid components in the cyst.  Also, it will help ease your mind and your pain.  Please let us know what you find out.

i have many of those symptoms and they are getting worse in my case also, a week from tomorrow i too have an appointment. this sounds like you have a cyst. during your gyno appointment you should make sure a ultrasound is done and also a urine culture. if they do not find anything push for different testing. sometimes cysts are able to hide. vaginal ultrasound is the best way to detect cyst so if i were you i'd get it done vaginally. i hope you feel better soon and you like all women going through this will be in my prayer, i wish you well and send you lots of love and support.

ur symptons i think are like i had pointing to a cyst best to get an appointment with a gyno they will know i wish you well hope you get to the bottem of ur problems
