dime sized bump on 2-yr old

I have a little sister who is two years old. we just barely noticed that she has a bump on her forehead (on the left temple)  about the size of a dime, nearly invisible. It has a small hole in the middle, and we don't think that it could be a bug bite because it is the middle of winter.

Is it itchy or painful does she rub it, we do get bugs in mild winters depending on where you live, has it been there long ? Has she fallen and bumped her head ?
