It depends if you did the registering process. Usually the first "appointment" is to get you all set up and to tell the dos and donts of pregnancy and filling out family history etc. If its the actual doctors appointment usually they do blood work and a pap smear. Most of the time they wont try for a heartbeat that early unless its a vaginal ultrasound because baby is so small and behind your pelvic bone.
At my first appointment they gave me a PAP (yay), and you do get to hear the heartbeat with a doppler. I got to ask the doctor any questions that I had at all. And also for my first appointment I was already 10w 4d so they gave me an ultrasound as well. Good luck & congrats!!! :D
General questions and paperwork probably. And you should be able to hear the heartbeat! I got a vaginal ultrasound at my first appointment at 6 weeks to confirm the pregnancy.
