First, hygiene has NOTHING to do with whether a person has an STD or not. You can't tell whether someone is more or less likely to have an STD based on how "clean" they look.
The chances of getting any STD from oral sex are much lower than getting an STD from having vaginal or anal sex. Receiving oral sex is a risk for gonorrhea, chlamydia, syphilis, HSV-1 (herpes type 1) and NGU. If you've ever had a cold sore, you already have HSV-1 and the chances of you getting it genitally are extremely low.
