With relation to HIV, only the head of your penis needs to be covered. the hole was on the side, so I'd venture to say no risk at all for HIV. It does sound like the other person is careful and has back up that she tests but as Grobick says, a condom break during intercourse would make it a good idea to get tested.

The fact your partner had STI test results is very reassuring: it means she is aware of STI risks, that is good for you.
It is also well-established in studies that high-profile CSWs are much more careful about protection and testing. That is also good news for you.
Sadly, it doesn't make your encounter safe. The general rule is "broken condom means unprotected sex". A single case of unprotected vaginal sex poses little risk for HIV.
I would say HIV should be the least of your worries. The risk is definitely not "substantial".
The test for gonorrhea and chlamydia is definitive at 14 days.
I would recommend abstain from sex with your wife.
I left out that during round 2 she got on top after the protected oral. The condom broke during intercourse. Not oral.
