I agree that your vitamin B-12 is way too low, as is folic acid... Deficiency of either of these can cause similar symptoms...
Seeing your results its does show hyperthyroid state as your Free T4 value is close to top threshold value.
Also you are severe deficiency in all 3 like B12, Folate and Vitamin D.
Hypercalcemia too can give such symptoms especially increased thirst and palpitations. You need to check these together when next time you do blood test.
