A blood test will tell you what your thyroid is doing. You have got to get the numbers down. The doctor will look at the lump also. You may have to have it out and a biopsy.

You need to get thyroid tested. And that means at LEAST these three things

1) TSH
2) Free T4 (FT4)
3) Free T3 (FT3)

In addition to Vit D I would also recommend you get tested for Vitamin B-12 and Iron and Ferritin  All of these will cause fatigue if too low in the range.

I doubt personally (I'm not a Dr.) that 1,000 IUs of D3 will significantly raise your Vit D levels much. Your D3 is quite if not very low!

Vit D is made by your body by sunlight on your skin.  And if you are from a northern climate especially in the winter, you are covered up and Vit D levels fall particualarly in the winter.

Some of your other symptoms are consistent with menopause so that could be a contributing factor.
