What is the best technique for separating a skin bridge?

What  technique offers the best results for the  separation  of a 3/4 inch Skin Bridge on the top right side of my penis?  

It is very tight when I am hard and it pulls my head slightly to the right. At times it does cause some mild discomfort and it has small tears in the skin in the center of the bridge. I can pass a Q-tip through it from end to end. I'm embarrassed to say that I am 49 years old and am only now seeing a urologist.

Can/will the doctor do the procedure in the office?  Is it very routine?

Well I went to see the urologist.   He agree that the best treatment would be to remove the skin from the head of the penis.  After he learned that I could pass a Q-Tip through, under and out the other end of the skin bridge be said that he would just place two clamps through the bridge and cut the skin and stitch both sides. He will do this in the office under a local.  He also indicate that he would have to remove the excessive skin. He said recovery time would be 3-4 weeks. He said that he might not be able to remove all the skin from the head. This concerns me. I hope I am making the right decision or should I just put up the the occasional irritation?
