Foreskin tear, how do I get it to heal completely?

I'm a 39 year old, uncircumsized and experienced this problem when I was having sex earlier last year.  Here's the deal, when I was having sex I noticed a slight pinch coming from my penis.  The next morning I noticed that a part of my foreskin was torn, maybe about 3 mm in length.  Now the tear is on the right side of my head about maybe 1 cm under the ridge of my head.

The first time it tore I used neosporin and it apparently healed well.  A couple weeks later (maybe 2 - 3) I felt the same pinch once again.  And I tried to clean it well as before and use neosporin.  This time it did not work as well as the first time.  I've tried another anti-biotic perscribed from my urologist but that didn't work.  I tried the scar medicine Medicam (sp?) and that apparently worked for a bit--it appeared healed but when having sex again I could feel the pinch coming on.

Now I don't know what to do about this because even trying to abstain from sex/masturbation for say a week to 2 weeks I can slightly feel it pinch.  I mean it seems I'm able to get the woman very lubricated but still I'm getting frustrated here.

Again its not the frenulum, the tear is on the right side of my penis.  However, I've noticed that when I masturbate I can control not having it tear or pinch.  And I've been able to just have sex w/o fully thrusting and concentrating on just penetration with the head of my penis and it goes well, but sometimes I want to go all in--which is when I start to feel it pinch.

Suggestions are greatly appreciated

Hello,
From the symptoms it looks that along with topical antibiotics, you need oral antibiotics also. Wash the areas several times with fresh water. Do not use any cosmetic products at the sites. You can apply some calamine lotion at the rash as it will help in soothing the skin.Also wear cotton undergarments.Application of mild corticosteroid cream and oral antiinflammatories like ibuprofen/acetaminophen are also helpful.

For this, I would suggest you to consult a urologist.
