Ruptured spermatic cord following retroperitoneal varicocelectomy


Doctor:
I would like to know if damage to the spermatic cord can cause repeated internal infection of the inguinal canal (due to spillage of sperm in the inguinal canal), and recurrent prostatitis. I'd also like to know how this damage can be imaged, and what treatment options are available.

In 1985 I had a retroperitoneal or inguinal varicocelectomy on my left side. (I am not certain of the exact location because the scar is long gone.)

In 1999, while straining during a bowel movement, a "bubble" the size of a small marble appeared at the then still visible surgical scar. I pushed on the bubble, and it burst beneath the skin. There was a feeling of liquid flowing. The next morning, I awakened with a very weak and painful erection pointing to the left. My genitals were black and blue, and the tissue around the base of my penis was swollen (it STILL is). I had a high fever and chills, urinary incontinence, severe pain between the scrotum and anus, and balantis of my penis.

I was turned away from the emergency room, and had to wait a week for my doctor to see me. He diagnosed prostatitis, gave me a one week supply of Bactrim and sent me packing. No cultures were taken. Two weeks later, I was sent to a urologist who I would later discover was suffering from advanced Altzheimers. By the time the urologist saw me, the discoloration was gone. I was given an rectal ultrasound of the prostate, which apparently located no problems. I was put on a quinilone antibiotic for a few weeks until I experienced damage to the achilles tendons in both ankles. (They did not rupture.) The urologist then told me there was nothing he could do for me, and released me. The urologist died a short time later, and I do not know who has his records.  Another urologist then looked at me and diagnosed fungal balantis and possible chronic prostatitis.  He could not find a hernia, and did not know what to make of my comments about the varicocelectomy.

I lost my job and was declared unfit to work and ineligible for unemployment insurance. I became dependent on my parents. I was bedridden, unable to sit upright more than 10 minutes without extreme pain. If I attempted to sit longer, I lost control of my bladder.

After six months of bed rest, I was able to take a part time job close to home. My pain and urinary problems continued, and have continued to this very day. My doctors had no ideas, wouldn't suggest any treatment, or send me to any other specialists.   For several years, I pleaded with my doctors to send me for a scan of my groin, suspecting a dissected anuerysm.  I don't like to try and diagnose myself, but no one was giving me any help.  All they would perform were XRays and Cat Scans without contrast. I finally lost hope of a diagnosis or cure, and have not seen a doctor in the last 5 years.

I have been dealing with the continuous daily pain, and discomfort, all these years, unable to sit up for more than a few minutes. Unable to travel, unable to sit in a restaurant, classroom, theatre or church pew. I have recurrent infections in which my groin area gets hot to the touch. My prostate gets inflamed and I cannot urinate. I have severe relapses periodically, always triggered by nocturnal emissions. I never recovered a proper erection. I wonder if the cause of these frequent attacks could be a ruptured spermatic cord that is leaking sperm or other bodily fluids into my inguinal canal, causing frequent attacks of prostatitis.

After all these years abscence, I have an appointment with a new PCP this week. I don't know how to talk to him about this.

I feel very sorry for you, im no doctor but can definetly understand your pain and anguish. I have an issue myself that never goes away and even had varicocelectomy. Nobody can find anything. Pain never really goes away, if im without the suspensory for 2 days.. Omg pain is unbearable. Right side, pressure and pain around top right side kind around the belt line and stomach, groin area, right testicle, loosen leg... Lower back pain, get to nausea and/or loosen stools, is like a huge combo of things at once. Ive been having all these issue since 2 years and nobody finds anything... The only little bit wierd finding was the varicocele, and the little bigger one wanst even on the right side.. The suggestion to operate was because of easy money for the doc. Even when he did open he admitted my veins werent that swollen and that probably it wanst the cause of my pain. Also, Nobody can find hernias . There's definetly aomething wrong.... So like i said I understand your anguish ..dont feel alone in this, very important.
