Soreness/swelling at tip of penis

Hello,

For nearly the past two years I have been struggling with a soreness and swelling at the tip of my penis that manifests itself almost daily.

An image is here:

(Please notice the swelling/enema right around the tip of the urethra)

The discomfort really seems to be the most noticeable at night and in the morning (potentially aggravated by nightime erections?), but the tip is still sensitive during the day. The easiest way to describe it is that it feels like my penis is bruised, but only at the tip. I have also experienced some discomfort during urination. I have noticed the easiest way to avoid the symptoms is to avoid erections/ejaculation so this has had a really negative effect on my libido.

While at the tip, the soreness doesn't quite feel topical like a skin irrititation (again, it's like a bruise)

I have been to a urologist and so far I have tried antibiotics and lotrisone, but nothing helps. I have also tried a few home remedies such as tea tree oil, moisturizing cream, and just cleaning in salt water, drying, and applying vaseline. Nothing seems to fix this issue and I long to just be the person I used to be.

If you need some background information, about 2 years ago I was going through a period of an unusually high libido and was masterbating very often. At one point I believe I masterbated a little too much because my penis got sore and the soreness never really went away (although it is better than it used to be).

This has really affected me on a personal level and I really appreciate any guidance you may have.

Thanks!
