Lump on left testicle

A few days ago my left testicle became sore to the touch and felt like it was being slightly pulled upwards..now I don't really have any pain from it but there is a lump on the front of my left testicle with a large tube coming out from it..this has never been there before and i'm wondering if it could be cancer or a Varicocele?

Thank you very much for taking the time to help me with this..the pain has pretty much gone away now but the growth/tubelike thing has gotten bigger and is now about half the size of my testicle. I am going to get advice from a urologist as soon as possible but it might be a few days. Are there any risks in waiting or should I go to an emergency room?

Hello,
There are 2 possibilities which I can think of in your case.
The first is an epididymal cyst which is a cystic swelling found in the upper part of the testes and it contains clear fluid and the other one is a spermatocoele which contains a milky fluid as it contains the sperms. I will suggest a scrotal ultrasound to be done and epididymal cysts are usually managed by doing a cyst removal surgically or by something known as sclerotherapy in which a sclerosant solution is instilled into the cyst and the cyst dries up.
However, these procedures are usually carried out only if the patient is symptomatic (e.g. any pain,discomfort).
